What we do
We offer early intervention, needs led support, through direct one to one work with children, young people, parents and carers. Our goal is to support a foundation to identify and confidently live the life they want as their authentic self. We offer:
Four one to one sessions with child/young person and give them a safe space to talk about their needs.
Exploring strengths, needs, and identity as an individual.
Helping build self-advocacy skills to confidently explain what support works best.
Initial parent/carer session to answer questions and share information about local support and how they might support their child.
Joint session (with permission) to help develop a shared understanding of neurodivergence.
Bradford: For children and young people, aged up to 18 years and their families who have received a diagnosis of autism and/or ADHD. The referral pathway is Bradford ND Assessment Team and can also come through the ND Family Navigator, Bradford.
Calderdale: For children and young people, up to 18 years and their families who may need early support focussed on the child/young person's needs. The referral pathway is self-referral from the First Point of Contact and Calderdale Open Minds teams. Referrals can also come through the ND Family Navigator, Calderdale.
Kirklees: For children and young people, up to 18 years and their families who may need some early support focussed on the child/young person's needs. The referral pathway is self-referral and may also come through the ND Family Navigator, Kirklees.
Leeds: For children and young people, up to 18 years and their families who have received a diagnosis of autism and/or ADHD. The referral pathway is Leeds ND Assessment Team and through the ND Family Navigator, Leeds and the Bramley Cluster/schools.
Wakefield: The referral pathway for this service is for students attending Horbury Academy Secondary School. The service is also open to self-referral.
